At Frontcare 247 Supported Living Services we believe that working closely with service users and in partnership with a variety of professional services is key in assisting the individuals placed in our care to reach their full potential. We have a highly detailed care planning system in place which is operated based on a thorough pre-placement assessment. As much information as possible about the service user is gathered to ensure we understand why the placement is being proposed and what the care needs are likely to be.

 

This way, we can develop with the service user, agreed targets and be more specific about the outcomes we want to see for them whilst being able to help service users address issues pertinent to their specific needs as reflected in their individual Care Support Plans.
